About the Role:

We are seeking a skilled Occupational Therapist to join our Reablement team, working in partnership with organisations across Health and Social care to maximise independence, prevent hospital admissions, and facilitate rapid discharge from hospital to improve outcomes for the people of Hampshire.

As a qualified Occupational Therapist with current HCPC registration, you'll be responsible for leading holistic assessments to address reablement needs and planning, implementing and reviewing interventions. You'll provide support across our multi-disciplinary team, working collaboratively to provide the best level of care to our service users.

Our Reablement service operates 7 days a week, and you'll be required to work weekends on a rotational basis. You'll also be expected to travel to various locations within Hampshire, so a current drivers' licence and access to a reliable vehicle are essential.

Key Responsibilities:
  • Lead holistic assessments to address reablement needs
  • Plan, implement and review interventions to support service users
  • Provide support across our multi-disciplinary team
  • Travel to various locations within Hampshire
