Support Worker

About the Role:
We are seeking a dedicated and compassionate Support Worker to join our team at Active Care Group. As a Support Worker, you will play a vital role in providing high-quality care and support to individuals with complex care needs, including those with brain injuries.
Key Responsibilities:
- To establish and maintain a trusting relationship with clients, their families, and case managers.
- To support clients in line with their care plans and promote their independence.
- To maintain a safe and secure environment, adhering to health and safety guidelines.
- To communicate effectively with all parties involved, including clients, families, and healthcare professionals.
- To contribute to the protection of individuals from abuse and neglect.
- To liaise with other agencies and professionals as required.
- To maintain accurate records, including time sheets, daily records, and care plan summaries.
Requirements:
- A commitment to providing person-centered care and support.
- Excellent communication and interpersonal skills.
- Previous experience in healthcare or social care, preferably with individuals with complex needs.
- Ability to work independently and as part of a team.
- Reliability, integrity, and trustworthiness are essential attributes for this role.
