Key Responsibilities
  • Develop and implement person-centred care plans to promote independence and well-being.
  • Provide emotional support and guidance to individuals, promoting their confidence and self-esteem.
  • Work collaboratively with colleagues and external agencies to ensure seamless care delivery.
  • Conduct risk assessments and develop strategies to mitigate risks, ensuring the safety and well-being of individuals.
  • Maintain accurate and up-to-date records, adhering to data protection and confidentiality policies.
Requirements
  • Diploma in Health and Social Care Level 2, with a willingness to work towards Level 3.
  • Minimum 3 years' experience in adult care, with a focus on learning disabilities and mental health.
  • Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and colleagues.
  • Ability to work flexibly, including evenings, weekends, and bank holidays.
  • Valid Enhanced DBS Disclosure and ability to work in the UK without sponsorship.
