About the Role

We are seeking an experienced Registered Nurse to join our Virtual Ward team as a Senior Clinical Lead. This is an exciting opportunity to be part of a pioneering initiative that is transforming the way we deliver care to patients in their own homes.

Key Responsibilities
  • Lead the development and implementation of virtual ward pathways, working closely with multidisciplinary teams to ensure seamless care transitions.
  • Assess and plan individualized patient care, utilizing digital health solutions to optimize patient outcomes.
  • Collaborate with stakeholders to promote effective use of community services and digital health tools.
  • Provide clinical leadership and mentorship to nursing staff, ensuring high-quality care delivery and adherence to clinical governance standards.
  • Contribute to the development of clinical policies and procedures, ensuring alignment with national guidelines and best practices.
Requirements
  • NMC Registered Nurse (Adult) or HCPC Registered Paramedic.
  • Recognized teaching or mentorship qualification at degree level or equivalent.
  • Management and leadership module or study days/proven evidence of.
  • Proven knowledge of Government policies relating to Virtual Ward and Clinical Governance.
