About the Role

We are seeking an experienced and skilled Registered Nurse to join our Virtual Ward team as a Senior Virtual Ward Nurse. This is an exciting opportunity to be part of a pioneering initiative that is transforming the way we deliver care to patients in their own homes.

Key Responsibilities
  • Assess and plan individualized patient care to meet the needs of patients with complex conditions.
  • Develop and implement digital health solutions to support patients with long-term conditions.
  • Work closely with multidisciplinary teams to ensure seamless care transitions and effective communication.
  • Provide specialist advice and support to patients, carers, and healthcare professionals.
  • Contribute to the development and implementation of virtual ward pathways and protocols.
Requirements
  • NMC Registered Nurse (Adult) or HCPC Registered Paramedic.
  • Recognized teaching or mentorship qualification at degree level or equivalent.
  • Management and leadership module or study days/proven evidence of.
  • Proven knowledge of Government policies relating to Virtual Ward.
  • Understanding of Clinical Governance.
