About the Role

The Leicestershire County Council Home Care Assessment and Reablement Team (HART) is seeking a dedicated and enthusiastic care professional to join our team as a HART Support Worker.

Job Summary:

The HART service helps individuals to return home from hospital or prevents them from being admitted by supporting them to regain their independence. As a HART Support Worker, you will provide short, intensive reablement support to people with disabilities and those who are frail and recovering from illness or injury, focusing on helping them to restore their optimum level of independence.

Key Responsibilities:

  • Provide short, intensive reablement support to individuals with disabilities and those who are frail and recovering from illness or injury.
  • Help individuals to regain their independence by supporting them with daily living tasks such as washing, dressing/undressing, showering, bathing, meal preparation, domestic activities, and engaging in community activities.
  • Support individuals with identifying suitable assistive technology equipment and demonstrating its safe use.
  • Advise on strategies for managing fatigue and pain, and preventing falls.
  • Work shifts on a rota that operates between 7.00am and 10.00pm, including evenings, weekends, and bank holidays.

Requirements:

  • Hold a Level 2/3 Diploma in Health and Social Care (or other relevant equivalent qualification) or an equivalent level of relevant experience.
  • Be committed to delivering high standards of care/support in an anti-discriminatory manner.
  • Be able to work effectively on your own as well as part of a team.
  • Have basic IT skills sufficient to write and maintain records.
  • Be able to communicate effectively both verbally and in writing and record relevant and accurate observations.
  • Be able to work under pressure and to remain calm in crisis situations.
  • Be willing to attend relevant job-related training as required.
