Job Overview

We are seeking a highly skilled and dedicated professional to join our Community Mental Health Team (CMHT) in Wolverhampton. As a key member of our team, you will play a vital role in providing high-quality, holistic care to individuals with complex mental health needs.

Main Responsibilities

You will be responsible for coordinating care for service users with complex, long-term mental health problems who face challenges in engaging with services. Your expertise will include assessing and formulating individual intervention plans, as well as developing outcome measures for adults with complex mental health needs. You will also participate in the CMHT duty system and manage the mental health needs of service users referred to the team.

Key Requirements

  • Registered Mental Health Nurse with experience in adult and community mental health services
  • Ability to assess and manage clinical risks, develop robust clinical management plans, and deliver a range of therapeutic interventions
  • Knowledge of the Recovery Model, psychotropic medication, and national quality indicators
  • Experience working in multidisciplinary teams and community settings
