We are seeking a Community Support Worker to join our team in North Somerset. As a Community Support Worker, you will play a vital role in providing person-centered care and support to service users, working closely with our multi-disciplinary team to deliver a range of clinical activities.
Key Responsibilities:- Undertake delegated clinical, therapy, and nursing-led activities to support service users on the Integrated Network Team caseload.
- Provide safe, patient-centered, and effective clinical activities in the community setting.
- Work within adult services to deliver a full range of healthcare activities.
- Be flexible and willing to work with other teams or services, both within the locality and Sirona wide, when the need arises at short notice.
- Travel throughout the service delivery geographical area.
