Job Title: Skilled Nursing Practitioner in the Community

As a skilled nursing practitioner in the community, you will be responsible for delivering high-quality care to patients in their own homes, while also collaborating with multidisciplinary teams to ensure seamless care coordination.

Key Responsibilities:
  • To carry out comprehensive assessments of patients' needs and develop individualized care plans.
  • Deliver skilled nursing care, including wound management, medication administration, and symptom relief.
  • Communicate effectively with healthcare professionals and community services to ensure optimal patient outcomes.

Requirements:
  • Band 5 or above registered nurse qualification
  • Valid driving licence and access to a vehicle
  • Recent experience (6 months) working as a community nurse
  • 2 clinical references covering 3 years of work experience
  • Valid DBS certificate
