As a Community Support Professional, you will be an integral part of our dynamic team, working collaboratively with colleagues and stakeholders to deliver high-quality support services to individuals with complex care needs.

In this fulfilling role, you will have the opportunity to develop meaningful relationships with our service users, understanding their unique challenges and aspirations, and providing tailored support to help them achieve their goals.

Your responsibilities will include:

  • Conducting assessments to identify the support needs of our service users, developing personalized plans to address these needs;
  • Providing practical and emotional support to service users, promoting independence, confidence, and positive relationships;
  • Liaising with other professionals, including healthcare teams, to ensure comprehensive support for our service users;
  • Contributing to the development of new initiatives and programs aimed at improving the lives of our service users;

We offer a competitive salary range of £28,000 - £32,000 per annum, plus benefits package, which includes:

  • 31 days' paid holiday per year, increasing with each year of service up to 33 days;
  • The option to buy additional holidays and spread the cost;
  • A comprehensive benefits package, including employee discounts, pension scheme, and access to training and development opportunities.
