Job Title: Support Worker Specialist

We are seeking a highly motivated and compassionate Support Worker Specialist to join our team at Together Trust Centre. This is an exciting opportunity for someone who is passionate about supporting young people with disabilities or complex health needs.

About the Role:

  • To provide specialist support to young people and their families in their own homes and communities, promoting independence and self-advocacy skills.
  • To work collaboratively with other professionals to develop and implement person-centred plans that meet the individual needs of each young person.
  • To contribute to a comprehensive package of training and development opportunities, including apprenticeship qualifications.
  • Annual salary: £23,500 - £28,000 per annum, depending on experience.
  • This role is ideal for someone who is enthusiastic, great communicator, and eager to provide high levels of support to children and their families.
