Active Care Group's Care in the Home division is a leading provider of safe, reliable, and outcome-based care to clients across the UK.

We are committed to delivering exceptional care and support to our clients, and we are seeking a highly skilled Care Coordinator to join our team.

The successful candidate will be responsible for coordinating and scheduling care packages to ensure continuity of care for our clients in their own homes.

Our care in the home services support individuals with a wide range of conditions, including brain injury, spinal cord injury, neurological conditions, and neurodegenerative diseases.

Key Responsibilities:

  • Inbound and outbound calls to clients and care workers to check welfare and shift cover
  • Updating the Team Leader on business-critical issues within the team
  • Maintaining compliance with company policies and procedures, ensuring regulatory standards are met
  • Providing and receiving complex, sensitive, and contentious information, and communicating effectively with staff at all levels
  • Identifying and collaborating on innovative ideas, demonstrating vision and supporting a creative, flexible, and responsive workplace
  • Developing and delivering best practice, ensuring workloads are met in a timely manner
  • Representing the company and maintaining positive relationships with internal and external stakeholders

Requirements:

  • Experience as a Care Coordinator
  • Previous experience of using booking and phone systems
  • Ability to deal with conflict in a calm and supportive manner
  • Strong work ethic and a dynamic, flexible approach to the role
  • Excellent customer service skills
  • Ability to work as a team, alone, and think outside the box
  • Excellent communication and interpersonal skills
  • Willingness to learn and develop
  • Enhanced DBS check
