About the Role
We are seeking a highly skilled and compassionate Social Prescriber to join our team at Southampton West Primary Care Network (PCN) Limited. As a Social Prescriber, you will play a vital role in reshaping the way we work with our patients to provide personalised care.
Main Responsibilities
You will work alongside our integrated team of Health & Wellbeing Coaches, Peer Support Workers, Mental Health Practitioners, and Care Coordinators to provide an all-encompassing approach to personalised care. Your key responsibilities will include:
- Undertaking holistic assessments and co-designing Health and Wellbeing Plans with individual service users.
- Developing trusting relationships by giving people time and focus on what matters to them.
- Providing service users with continuity and a coordinated experience of care, remaining a point of contact throughout the individual's social prescription.
- Working in partnership with all voluntary and community organisations to build a comprehensive database of local resources to design and support the Social Prescription Menu.
Requirements
To be successful in this role, you will need:
- Excellent interpersonal skills and the ability to work non-judgmentally with a wide cohort of people.
- High levels of emotional intelligence and the ability to work autonomously.
- Experience in giving Health advice to individuals and a proven understanding of safeguarding for children and vulnerable adults.
- Excellent holistic assessment and consultation skills, as well as the ability to provide empowering support to adults in a planned and structured way to improve health, recovery, and wellbeing outcomes.

We offer a competitive salary of £27,000 per annum (full-time equivalent) and the opportunity to work in a fast-paced and rewarding environment. You will also have access to a range of benefits, including the NHS Pension Scheme and a comprehensive training programme.
