Job Summary: Pharmacist in Community Care

We are seeking a dedicated and compassionate pharmacist to join our team and provide high-quality clinical pharmacy services to the South Kent Coastal community mental health team. As a Pharmacist in Community Care, you will be responsible for delivering safe and effective medicines management services within your Community Mental Health Team.

You will work closely with GPs, primary care pharmacists, and pharmacy technicians, as well as the KMPT pharmacy team and clinicians in secondary care. You will also be required to network and drive innovation in mental health medicines optimisation across the primary and secondary care interface.

Main Responsibilities:
  • Provide high-quality, patient-centred care to community-based patients and their carers within Kent and Medway NHS and Social Care Partnership Trust (KMPT).
  • Work collaboratively with GPs, primary care pharmacists, and pharmacy technicians to deliver safe and effective medicines management services.
  • Develop and implement strategies to improve patient outcomes and reduce healthcare costs.
Requirements:
  • Master's Degree in Pharmacy
  • Member of the General Pharmaceutical Council, with at least 3 years' post-graduate experience
  • Postgraduate qualification in Clinical Pharmacy or equivalent specialist knowledge
  • Non-Medical Prescribing qualification or working towards this qualification

Salary: £65,000 - £85,000 per annum, depending on experience
