Job Description

We are seeking an experienced and skilled Discharge Planning Specialist to join our team at MK Together. As a key member of our Integrated Discharge Hub, you will play a vital role in ensuring seamless transitions for patients between hospital and community services.

Main Responsibilities
  • Discharge Planning: Work closely with multidisciplinary teams to assess patient needs and develop personalized discharge plans.
  • Coordination: Coordinate with healthcare professionals, social services, and community providers to ensure timely and safe discharges.
  • Communication: Maintain effective communication with patients, families, and healthcare teams to ensure a smooth discharge process.
  • Problem-Solving: Identify and resolve barriers to discharge, working collaboratively with the care team to find solutions.
Requirements
  • Qualifications: HCPC registration and a registered professional qualification in occupational therapy or physiotherapy.
  • Experience: Proven experience in clinical or operational service management, with a focus on discharge planning and coordination.
  • Skills: Effective leadership, communication, and problem-solving skills, with the ability to work independently and as part of a team.
