We are seeking an experienced Registered Nurse/Advanced Nurse Practitioner/Advanced Clinical Practitioner/Paramedic to join our Virtual Ward team as a Senior Practitioner.

The Virtual Ward is a new and innovative integrated service that aims to support people to receive the care they need at home, rather than in hospital. As a Senior Practitioner, you will be responsible for undertaking advanced clinical assessments of patients on the Virtual Ward and developing, implementing, and evaluating care and treatment plans.

Key responsibilities include:

  • Undertaking advanced clinical assessments and developing care and treatment plans
  • Ensuring the day-to-day operational delivery of high-quality care for patients on the Virtual Ward
  • Leading and working with multidisciplinary teams to create clinical management plans
  • Supporting the development and delivery of new national initiatives in relation to virtual ward
  • Coordinating patient pathways within the Virtual Ward
  • Remote monitoring of patients across various clinical pathways
  • Ensuring effective referral pathways for the Virtual Ward, including initial triage
  • Providing patients with physical health and biopsychological assessments
  • Enabling timely discharge of patients from the Virtual Ward

Person specification:

  • Relevant professional qualification and current registration
  • Advanced assessment and history-taking course or relevant clinical experience
  • Mentorship or willingness to work towards

Desirable qualifications include a non-medical prescribing qualification (V300) and experience of practice development, clinical education, or leading change in practice.

Essential experience includes relevant post-registration experience, experience of working in community/acute settings, and experience of developing relationships with external agencies.

Other essential requirements include the ability to drive and have access to a car for work purposes.
