About the Role

We are seeking a dedicated and compassionate individual to join our team as an Empathetic Care Practitioner. This role is ideal for someone who is passionate about empowering individuals with mental health issues and learning/physical disabilities to live more independently.

Job Description

In this position, you will work under the guidance of the Senior Support Worker, providing practical and emotional support to adults with various mental health conditions. You will be working within our accommodation-based services in Milton Keynes, with a small group of adults who either live on their own with 1:1 staff support or in a small group setting.

  • You will make a huge difference to our clients' lives by aiding them in basic duties, including supporting access to local community activities, accompanying them to appointments, administering medication, budgeting, enhancing living skills, and providing evening meals from a set menu.
Key Responsibilities
  • Care Planning: Assess client needs, develop care plans, and implement supportive strategies.
  • Mental Health Support: Provide emotional support, encourage participation in activities, and promote independence.
  • Medication Management: Administer medication according to instructions and support clients with self-management.
  • Household Management: Assist clients with daily tasks, including laundry, cleaning, and meal preparation.
Requirements

To be successful in this role, you will need:

  • A strong understanding of mental health issues and learning disabilities.
  • Excellent communication and interpersonal skills.
  • The ability to work flexibly and adapt to changing situations.
  • A willingness to learn and develop your skills through extensive training.
