Job Title: Senior Practitioner - Virtual Ward

We are seeking an experienced Registered Nurse/Advanced Nurse Practitioner/Advanced Clinical Practitioner/Paramedic to join our team as a Virtual Ward Senior Practitioner.

About the Role

The Virtual Ward is a new and innovative integrated service that aims to support people to receive the care they need at home, rather than in hospital. As a Virtual Ward Senior Practitioner, you will be responsible for undertaking advanced clinical assessments of patients, developing and implementing care and treatment plans, and ensuring the day-to-day operational delivery of high-quality care.

Key Responsibilities
  • Undertake advanced clinical assessments of patients and develop, implement, and evaluate care and treatment plans.
  • Ensure the day-to-day operational delivery of high-quality care for patients admitted to the Virtual Ward.
  • Lead and work with multidisciplinary teams to create clinical management plans.
  • Support the development and delivery of new national initiatives in relation to virtual ward.
  • Coordinate patient pathways within the Virtual Ward.
  • Monitor patients remotely across various clinical pathways.
  • Establish an effective referral pathway for the Virtual Ward, including initial triage.
  • Provide patients with a physical health and biopsychological assessment.
  • Plan for and enable timely discharge of patients from the Virtual Ward.
  • Provide a high-quality support service to patients on the Virtual Ward caseload.
Person Specification

We are looking for a candidate with relevant professional qualifications, current registration, and experience of working in community/acute settings. You should have a strong knowledge base, supported by educational development, and be able to drive and have access to a car for work purposes.
