Job Description

The role of a Care Coordinator for Diabetes at Chemistree Solutions is an exciting opportunity to work in primary care, delivering high-quality patient outcomes and driving best practice. As a specialist nurse advisor with intimate knowledge of national treatment guidelines, you will work closely with patients and practices to assist healthcare professionals in delivering up-to-date care.

This role involves working across Northamptonshire, with a defined geographical area requiring excellent planning and organisational skills. The ideal candidate will be a patient-focused NMC registered nurse with experience of working in Diabetes, possessing strong interpersonal skills and a keen desire to deliver results with a consistent patient focus.

Responsibilities:

  • Delivering nursing programs to drive best practice and improve patient outcomes
  • Working in partnership with the NHS in primary care
  • Assisting healthcare professionals in delivering up-to-date care
  • Providing education, support, and advice to people with Type 2 Diabetes
  • Optimising oral hypoglycaemics and providing IT literacy support

Requirements:

  • NMC registration and a full UK driving license
  • A minimum of 3 years experience managing diabetes within primary care
  • Knowledge and skills in the optimisation of oral hypoglycaemics and IT literacy
