About the Role

We are seeking a highly skilled Primary Care Nurse Specialist to join our team at Chemistree Solutions. This is an exciting opportunity to work in the field of Diabetes, supporting healthcare professionals and patients in making informed decisions about treatment pathways.

The successful candidate will have extensive experience of working in primary care, possessing a deep understanding of national treatment guidelines. They will work closely with patients and practices to deliver high-quality patient outcomes and drive best practice.

Key Responsibilities:

  • Delivering nursing programs to drive best practice and improve patient outcomes
  • Working in partnership with the NHS in primary care
  • Providing education, support, and advice to people with Type 2 Diabetes
  • Optimising oral hypoglycaemics and providing IT literacy support

Requirements:

  • NMC registration and a full UK driving license
  • A minimum of 3 years experience managing diabetes within primary care
  • Knowledge and skills in the optimisation of oral hypoglycaemics and IT literacy
