About the Role


We are seeking an experienced Nurse Manager to join our team in Swansea Bay University Health Board, responsible for managing patients with complex care needs. As a key member of our Continuing Health Care Team, you will work closely with other healthcare professionals to provide high-quality patient care.



Job Responsibilities



  • Manage a caseload of patients with complex health needs, ensuring timely and effective delivery of care.
  • Lead and develop a multi-skilled healthcare team, promoting excellence in patient care and quality improvement.
  • Collaborate with other agencies and services to ensure seamless care coordination.
  • Develop and implement patient-centered care plans, working in partnership with patients, relatives, and carers.
  • Maintain accurate records and reports, providing data-driven insights to inform care decisions.
  • Ensure compliance with relevant policies, procedures, and regulatory requirements.


Required Skills and Qualifications



  • First-level registered nurse qualification.
  • Post-registration experience in managing patients with complex care needs.
  • Strong leadership and management skills, with ability to motivate and develop teams.
  • Excellent communication and interpersonal skills, with ability to work collaboratively with multidisciplinary teams.
  • Knowledge of community nursing and primary care principles, including caseload management and quality improvement.
