Job Summary

An exciting opportunity has arisen for a Band 6 Community Complex Care Sister to work in the Continuing Health Care Team at Swansea Bay University Health Board.

Main Responsibilities
  • Manage a team of Health Care Support Workers to provide high-quality care to patients with complex needs.
  • Develop and implement care plans to meet the individual needs of patients.
  • Work collaboratively with other healthcare professionals to ensure seamless care delivery.
  • Provide leadership and guidance to the team to achieve high standards of care.
  • Monitor and evaluate the effectiveness of care packages and make recommendations for improvement.
  • Collaborate with the Medicines Management Team to ensure safe and effective medication management.

Requirements
  • First-level registered nurse with post-registration experience in managing patients with complex care needs.

  • Excellent communication and interpersonal skills.
