Nurse Specialist in Palliative Care

We are seeking a dedicated and experienced Senior Nurse to lead our community-based dementia service in Swansea, West Glamorgan. As a key member of our team, you will be responsible for delivering high-quality, patient-centered care and contributing to innovative practice development.
Key Responsibilities- Lead educational initiatives to enhance staff knowledge and skills in palliative care
- Contribute to the development of new practices and protocols to improve patient outcomes
- Deliver high-quality patient assessments and care planning in collaboration with multidisciplinary teams
- Manage a small team of staff to ensure effective delivery of care services
- Promote a culture of excellence and continuous improvement within the team
- Registered Nurse on the NMC Register with a relevant Nursing Degree
- Proven experience in palliative care, preferably in a community-based setting
- Strong communication and team collaboration skills
- Ability to manage workload and prioritize effectively
- Experience in both formal and informal teaching and leading nursing practice developments
- Annual leave allowance of 27 days plus 8 public holidays (pro rata)
