Care Coordinator for Learning Disabilities

We are seeking a skilled and compassionate Nursing Associate to join our Community Learning Disability Team in Teesside. As a key member of our multidisciplinary team, you will work closely with individuals who require specialist interventions using trauma-informed care and a recovery-based approach.
The ideal candidate will have a genuine desire to improve the mental health and well-being of adults with learning disabilities and behaviors that challenge. You will be motivated, flexible, and able to communicate effectively, with a strong ability to work independently as well as part of a wider team.

Key Responsibilities- To provide high-quality secondary care and enhance community services for individuals with complex needs.
- To work collaboratively with multidisciplinary teams to deliver person-centered care and treatment plans.
- To travel independently to various locations within the NYY&S locality on a regular basis.
Salary: £26,530 to £29,114 per annum (pro rata)
