Key Tasks:
- Develop and maintain positive relationships with the individual, their family, and healthcare professionals.
- Assess and address the individual's physical, emotional, and social needs.
- Implement care plans that promote the individual's independence and well-being.
- Monitor and record the individual's health and well-being.
- Maintain accurate records of care provided and report any changes in the individual's condition.
Required Skills:
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Previous experience in a similar role, preferably in complex care.
