Job Description:

Creative Support is a not-for-profit organisation which provides high-quality, person-centred support to people with physical disabilities, learning disabilities, mental health needs and dementia. We are seeking a warm, compassionate and proactive Care Coordinator to provide person-centered care and support to our service users.

Key Responsibilities:

  • Provide person-centered care and support to service users
  • Assist the Extra Care Registered Manager and Deputy Manager in the operational management of the service
  • Collaborate with the Extra Care Registered Manager, Deputy Manager, the housing provider, NHS professionals, Social Services and other stakeholders to ensure that the highest standards of care are provided
  • Work in conjunction with service users' families and friends to promote a positive atmosphere within the service
  • Ensure that the care provided empowers clients and meets their needs

Requirements:

  • Previous experience in services for older people, people with dementia and memory loss
  • Warm, person-centered, and respectful approach
  • Ability to work in an empathetic and holistic way to maintain orientation and independence
  • Flexibility, energy, and reliability
