About the Role

We are seeking a highly skilled and dedicated Practice Pharmacist to join our team at Pearl Medical Centre in Birmingham. As a key member of our proactive and forward-thinking practice, you will play a vital role in providing high-quality patient care and contributing to the success of our team.

Key Responsibilities
  • Conduct medication reviews and optimise medication regimens for patients with complex needs.
  • Process hospital discharge letters and out-patient treatment requests from specialists.
  • Lead on forming and maintaining relationships with community pharmacists and other service providers.
  • Communicate effectively with clinicians of all disciplines on complex medication issues.
  • Act as an advocate and care co-ordinator for patients with complex pharmaceutical needs.
Requirements
  • Pharmacy degree and registration with the GPhC.
  • Independent prescribing qualification and evidence of commitment to continuing professional development.
  • Previous experience in a GP practice and/or hospital environment is highly desirable.
