Main Duties and Responsibilities

This is a chance to get involved in a Practice that works democratically, with a strong focus on providing high-quality patient care. Previous experience in Primary Care is highly desirable, but not essential. We are looking for a motivated and proactive individual who is eager to make a positive impact on patient care.

About Us

Pearl Medical Centre is a dynamic and patient-focused Practice with a list size of circa 10,000. We are an EMIS Web and Docman practice, with a paper-light approach. We are a high QOF achiever and have a dedicated team of healthcare professionals, including GPs, Practice Nurses, Nurse Prescribers, Pharmacist Prescribers, and HCAs.

Key Responsibilities
  • Undertake and record medication reviews with direct patient contact for patients on complex medicine regimes/at high risk of harm from medicines.
  • Proactively liaise with clinical staff to ensure this service is offered to patients who would benefit from having their medicines optimised.
  • Record interventions made during medication reviews using template provided and details of interventions that may have prevented an admission.
  • Challenge requests made to prescribe medication outside of local or national guidance.
  • Undertake regular medication-related audits and implement any actions identified.

  • Pharmacy degree
  • Registered with the GPhC
  • Independent prescribing qualification
  • Evidence of commitment to continuing professional development

Desirable

  • Previous experience in a GP practice and/or hospital environment.
