About the Role

We are seeking a highly skilled and dedicated Practice Pharmacist to join our team at Pearl Medical Centre. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and optimising medication use.

Key Responsibilities
  • Conduct medication reviews to ensure patients are receiving the most effective and safe treatment.
  • Process hospital discharge letters and out-patient treatment requests from specialists.
  • Lead on forming and maintaining relationships with community pharmacists and other service providers.
  • Communicate effectively with clinicians of all disciplines on complex medication issues.
  • Act as an advocate and care co-ordinator for patients with complex pharmaceutical needs.
Requirements
  • Pharmacy degree.
  • Registered with the GPhC.
  • Independent prescribing qualification.
  • Evidence of commitment to continuing professional development.
