Job Summary

The Royal Devon and Exeter NHS Foundation Trust is seeking a highly skilled and motivated Clinical Lead to join our Hospital Discharge Team. As a key member of our multidisciplinary team, you will play a crucial role in managing and supporting healthcare clinicians in complex discharge planning.

About the Role

This exciting opportunity is focused on fostering good relationships with care home providers and supporting smooth transitions from hospital to care home environments. You will work closely with our team to develop and implement effective discharge plans, ensuring that patients receive the best possible care and support.

Key Responsibilities
