About the Role
Triangle Housing Association is seeking a highly skilled and dedicated individual to join our team as a Senior Care Coordinator. As a key member of our support services team, you will play a vital role in delivering high-quality personal care and housing support services to individuals with a learning disability/challenging behaviour.
Key Responsibilities
- Work collaboratively with service users and staff to deliver quality care and support services
- Develop and implement individualised care plans to meet the unique needs of each service user
- Provide guidance and support to staff to ensure the delivery of excellent care and support services
What We Offer
- A competitive salary scale of Band 3 Level 1 - 4 (£24,695 - £25,853 per annum)
- 33 days annual leave (includes public holidays)
