Job Title: Community Response Support Worker
Job Summary:
We are seeking a skilled and compassionate Community Response Support Worker to join our team at DCHS Foundation Trust. As a key member of our Community Response Team, you will play a vital role in providing high-quality care and support to individuals in their own homes.
Key Responsibilities:
- Provide individualized care and support to patients in their own homes, promoting independence and optimal health outcomes.
- Work collaboratively with clinicians and other healthcare professionals to deliver agreed programs of treatment and care.
- Develop and maintain a range of knowledge and competencies to support clinicians in delivering high-quality patient care.
- Assist clinicians in delivering care and treatment programs, working as an effective member of the multi-disciplinary team.
- Document patient care and progress to a high standard, reflecting the DCHS values.
Requirements:
- UK driving license and access to a vehicle (mileage policy and car lease scheme available).
- Ability to meet travel requirements and work in a variety of settings.
- High standard of documentation and communication skills.
- Commitment to working in a way that reflects the DCHS values.
