Role Overview

NHS Dorset is seeking an experienced professional to join its Personal Health Commissioning Team as a Senior Healthcare Specialist - Continuing Care Management.

This role will involve managing a caseload of CHC eligible individuals in accordance with the National Framework for Continuing Healthcare, collaborating with healthcare professionals, community services, and acute services.

The ideal candidate will have extensive knowledge of specialist areas acquired through experience and training, as well as good understanding of Personal Health Commissioning and negotiation and conflict management skills.

A full driving licence and access to a vehicle for business use are essential requirements.

Job Responsibilities

  • Manage a caseload of CHC eligible individuals in accordance with the National Framework for Continuing Healthcare.
  • Collaborate with healthcare professionals, community services, and acute services to provide high-quality assessments and timely service delivery.
  • Work flexibly and autonomously, making informed decisions in the absence of required information, working to tight and often changing timescales.
  • Make off-site visits to undertake assessments and attend meetings.

Requirements

  • Educated to degree level in a relevant subject or equivalent level of experience working at a similar level in a specialist area.
  • Nursing qualification and registered with a professional body.
  • Good understanding of Personal Health Commissioning and associated procedures and practices.
  • Evidence of Continued Professional Development.

Salary Information

The estimated salary for this role is around £40,000 - £60,000 per annum, depending on experience and qualifications.
