Job Summary

We are seeking a clinician with a specialist interest in enhanced diabetic care to join our team. You will work in conjunction with our well-established practice nurse and clinical teams to provide high-quality care to our patients.

Main Responsibilities

  • Provide a first point of contact within the practice for the management of diabetic patients
  • Proactively monitor the care of all diabetic patients in line with local and national policy
  • Prescribe medication, including insulin, and manage patients appropriately
  • Support the clinical team in the development of individual treatment plans for patients with long-term conditions
  • Request pathology services and process pathology results as required
  • Provide specialist advice and knowledge to newly diagnosed diabetic patients and their families
  • Undertake annual diabetic reviews autonomously, commencing/altering treatments as necessary
  • Deliver patient care as necessary, referring patients to secondary/specialist care as required
  • Maintain accurate clinical records using ARDENS and SystmOne templates

Requirements

  • Diploma or equivalent in diabetes management with knowledge of Insulin and GLP-1
  • Professional registration with NMC, GPHC, or equivalent
  • Independent Prescriber
