About the Role

Uppingham Surgery is seeking a skilled clinician to join our team and provide enhanced diabetic care to our patients. As a key member of our clinical team, you will work closely with our practice nurse and other healthcare professionals to deliver high-quality patient care.

Key Responsibilities

  • Provide a first point of contact for diabetic patients and their families, offering specialist advice and knowledge to empower them to take control of their illness.
  • Proactively monitor the care of all diabetic patients, ensuring that their treatment plans are up-to-date and effective.
  • Prescribe medication, including insulin, and manage patients' conditions accordingly.
  • Support the clinical team in developing individual treatment plans for patients with long-term conditions.
  • Request pathology services and process results as required.
  • Deliver patient care as necessary, referring patients to secondary or specialist care when required.
  • Maintain accurate clinical records using ARDENS and SystmOne templates.
