Job Summary

Uppingham Surgery is seeking a clinician with a specialist interest in enhanced diabetic care to join our team. As an Enhanced Diabetes Clinician, you will work in conjunction with our well-established practice nurse and clinical teams to provide high-quality care to our patients.

Key Responsibilities

  • Provide a first point of contact within the practice for the management of diabetic patients
  • Proactively monitor the care of all diabetic patients in line with local and national policy
  • Prescribe medication including insulin and manage patients appropriately
  • Support the clinical team in the development of individual treatment plans for patients with long term conditions
  • Request pathology services and process pathology results as required
  • Provide specialist advice and knowledge to newly diagnosed diabetic patients and their families
  • Undertake annual diabetic reviews autonomously and deliver patient care as necessary
  • Maintain accurate clinical records using ARDENS and SystmOne templates
