Job Overview:

The Care Manager Professional at Commonwealth Senior Living assists the Resident Care Director in planning, implementing, and evaluating resident care to ensure high-quality care and experience for each resident. The role utilizes the nursing process to perform nursing functions according to Federal and State regulatory licensing requirements.

About You:

We are seeking a compassionate and detail-oriented Care Manager Professional to join our team. As a key member of our Resident Care department, you will work closely with residents, families, and support departments to plan for resident care needs.

Key Responsibilities:

  • Maintain liaison with residents, families, and support departments to plan for resident care needs.
  • Ensure provision of services to residents and validate compliance with Federal and State policies governing resident care.
  • Perform competent direct care and supervisory responsibilities.

Requirements:

  • Graduate of a state-approved school of practical nursing or current active state certification in medication administration.
  • At least one year of successful nursing experience preferably.
  • Work experience should include administration of medication and experience in geriatric, psychiatric, or rehabilitation nursing.

Estimated Salary: $65,000 - $75,000 per year.
