Job Title: Young Carer Assessor Practitioner

Description:

We are seeking a dedicated and compassionate individual to join our Integrated Front Door service as a Young Carer Assessor Practitioner. In this role, you will play a vital part in identifying the specific needs of young carers and coordinating support through commissioned services.

Key Responsibilities:

  • Conduct assessments of young carers under 18 who are caring for loved ones facing health challenges or substance misuse.
  • Collaborate with families and professionals to develop tailored support plans that prioritize the well-being, protection, and development of young carers.
  • Work closely with local authorities and service providers to ensure seamless access to commissioned services.

Requirements:

  • Degree in Social Work, Child Care, or a related field.
  • Experience working with vulnerable children and young people.
  • Excellent communication and assessment skills.
  • Ability to work autonomously and as part of a team.

Salary and Benefits: £25,000 - £30,000 per annum, comprehensive training and development programme, and a supportive team environment.
