Job Overview

The Hampshire Hospitals NHS Foundation Trust is seeking a highly skilled professional to join the Frailty Team in the Emergency Department and Acute Assessment Unit. As a Frailty Enhanced Practitioner Specialist, you will work closely with the community partners and be actively involved in the Hampshire Together programme.

Main Duties of the Job

You will be responsible for assessing patients' physical, cognitive, and social abilities to remain at home or put plans in place to meet their acute or long-term needs. You will work flexibly across hospital, community, and ambulance services in line with the evolving integrated care strategy.

Key Requirements

  • A passion for working with older adults
  • An understanding of the unique challenges someone living with frailty faces, especially when acutely unwell
  • Excellent communication skills
  • Previous experience with older adults or those with complex needs
  • Advanced assessment and care planning skills using a holistic approach
  • A willingness and ability to work hands-on in all aspects of patient assessment and care
  • Completed or looking to complete post-graduate education related to Frailty
