Rapid Response Care Coordinator

We are looking for a skilled Rapid Response Care Coordinator to join our community healthcare team. As a key member of our multi-disciplinary team, the successful candidate will provide comprehensive specialist assessment, review, case management, and evaluation of the needs of patients and carers. The ideal candidate will have experience of working in a community/in-patient setting and possess excellent presentation skills.
The salary for this role is estimated at £42,000 - £52,000 per annum, depending on experience.
