Buckinghamshire Healthcare NHS Trust is delighted to announce a Band 6 District Nurse/Community Nurse/Case Manager position, offering a competitive salary of £42,000 - £52,000 per annum.

The role is part of the Marlow Adult Community Health Care Team (ACHT), which reflects an integrated approach designed in response to the changing needs of the local population. The team leadership consists of a Community team lead, an Advanced Case Manager with Clinical Practice Teacher qualifications, and another advanced case manager with extensive team management experience.

The ACHT's emphasis is on proactive care for patients in the community setting, including caseload management, prevention of admission, reablement, and end-of-life care. In addition, successful candidates will facilitate timely discharge from hospital through coordinated care approaches with key care partners.

Key responsibilities include:

  • Implementing tailored interventions based on individual patient needs within the case management model
  • Contribute to processes identifying high-intensity users of community and primary healthcare services at risk of unplanned hospital admission
  • Assess physical and psycho-social needs of defined caseloads, instigating therapeutic treatments and care delivery based on best available evidence
  • Enhance health outcomes using specialist nursing skills and knowledge
  • Undertake referral management to develop case management programs for patients referred to the Adult Community Healthcare Teams

To excel in this role, you must have:

  • A registered allied healthcare professional qualification or registered level 1
  • Nurse degree/diploma in health/nursing studies or related subject
  • Evidence of recent continuous professional development
  • Relevant postgraduate clinical training in areas such as elderly, long-term conditions, respiratory disease, cardiac disease, dementia, rehabilitation, trauma, orthopaedics, and end-of-life care
