About the Role

As a Community Health Nurse Specialist at Harrogate and District NHS Foundation Trust, you will be responsible for delivering and leading highly specialist care within the Community Care Team. This role is an exciting opportunity to make a real difference in the lives of patients.

Main Responsibilities
  • Ongoing management of a defined caseload
  • Accepting referrals from health and social care professionals, voluntary organisations, and self-referrals
  • Leading the organisation, management, and development of the Community Care Team
  • Assessment, implementation, evaluation, and reassessment of care for all patients referred to the team
  • Planning programmes of care for patients with diverse and complex health needs

The post holder will work closely with partner organisations to support and oversee the assessment, planning, and delivery of personalised care. The ideal candidate will have strong leadership and management skills, as well as excellent clinical knowledge and assessment skills.
