About the Role

We are seeking a highly skilled Senior Community Health Specialist to join our team in Harrogate. This is an exciting opportunity to work with our dedicated team in providing physical healthcare for patients within the adult Mental Health team.

As a Senior Community Health Specialist, you will have a key role in carrying out physical health checks and overseeing weekly clozapine clinics and support for High Dose Antipsychotic Treatments (HDAT). You will also be responsible for supporting service improvement in the area of physical health monitoring and improvement amongst our patient population.

You will work closely with the multidisciplinary team to provide a high-quality service to patients through support, intervention, and clinical expertise on various aspects of physical health, including assessment, diagnosis, and review, chronic illness management, management of acute presentations, and ongoing physical health monitoring.

**Salary:** £43,041 - £59,275 per annum depending on experience

**Required Skills and Qualifications:*

  • Evidence of Continuing Professional Development
  • Key skills in literacy, numeracy, and IT (Q level 2 or equivalent)
  • Post-graduate qualification in a relevant area
  • Significant experience working with people with mental ill health in a care environment
  • Providing clinical supervision
  • Quality improvement activities
  • Demonstrable knowledge to post-graduate level of evidence-based practice in caring for patients in the designated field
  • Clinical Risk Assessment and Management and its application in practice

This is a full-time position that requires independent travel across the locality and access to a suitable vehicle for business purposes.
