Job Overview

We are seeking a skilled Clinical Health Specialist to join our team in Harrogate and Ripon Integrated Community Team. This is an exciting opportunity to work with us in providing physical healthcare for patients within the two adult Mental Health teams.

Key Responsibilities:
  • To carry out physical health checks and have an oversight of weekly clozapine clinics and support for High Dose Antipsychotic Treatments (HDAT).
  • To support service improvement in the area of physical health monitoring and improvement amongst our patient area.
  • To work closely with the MDT to provide a high quality service to patients through support, intervention and clinical expertise on the various aspects of physical health including: assessment, diagnosis and review, chronic illness management, management of acute presentations and ongoing physical health monitoring.
Requirements:
  • Nursing applicants should already hold NMC registration and have successfully completed their preceptorship programme.
  • Newly qualified Nurses or those due to qualify will be recruited via a separate process.
  • Evidence of Continuing Professional Development.
  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent).
  • Post-graduate qualification in a relevant area.
  • Significant experience working with people with mental ill health in a care environment.
  • Providing clinical supervision.
  • Quality improvement activities.
  • Demonstrable knowledge to post-graduate level of evidence-based practice in caring for patients in the designated field.
  • Clinical Risk Assessment and Management and its application in practice.
  • The Trust's Quality Improvement System (QIS).
