The Community Ambulatory Intravenous Clinic offers a comprehensive service for residents of Hillingdon, providing intravenous administration across the borough. With referrals mainly from local hospitals, the service operates 7 days a week at Oak Farm clinic in Hillingdon. While primarily clinic-based, home visits may be necessary to provide community intravenous medications to patients.

Job Description:

We are seeking a skilled Band 6 Nurse to join our team as a Community Health Specialist. The successful candidate will work closely with patients, families, friends, and carers to provide high-quality care.

  • Ensure patient safety through sound clinical skills and effective risk assessments.
  • Deliver best possible clinical outcomes by utilizing up-to-date skills and adhering to evidence-based policies and procedures.
  • Improve patient experience by respecting, empowering, and working in partnership with individuals throughout the care planning process.
Responsibilities:
  • Use clinical judgement and risk assessments to keep patients safe.
  • Safeguard people by recognizing and responding to potential abuse or harm, while also acknowledging personal limitations and escalating concerns when necessary.
  • Promote practice development, devising guidelines and procedures to support care delivery.
  • Reflect on practice to identify areas for improvement in safety and quality.
  • Maintain compliance with mandatory training requirements.
  • Ensure supplies and equipment are in place and maintained according to best practices and local policies.

This role is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, requiring Disclosure checks through the Disclosure and Barring Service.
