Job Description:
  • Deliver High-Quality Nursing Care:
    • You will work autonomously in patients' homes and community settings, ensuring safe and effective care delivery.

    • You will manage a daily caseload, prioritising tasks and responding to changes in patients' conditions.

  • Work Collaboratively:
    • You will work closely with other healthcare professionals, such as GPs, therapists, social workers, and voluntary sector partners, to ensure coordinated care.

    • You will participate in multidisciplinary team meetings and case conferences to support care planning and review.

  • Develop Patients' Skills:
    • You will provide health education and advice to patients and carers to support self-management and promote independence.

    • You will work with patients who have acute, chronic, or complex health conditions.

  • Manage Caseload:
    • You will use digital tools and systems to plan, record, and report on care delivery in line with organisational policies.

Requirements:

  • Registered Nurse (Adult) with current NMC registration

  • Clinical experience in a relevant setting, such as community, primary care, or acute services

  • Ability to work independently and manage a caseload in a community environment

  • Strong clinical assessment and decision-making skills

  • Excellent communication, interpersonal, and organisational skills

  • A full UK driving licence and access to a vehicle for work purposes (if applicable)

  • A commitment to patient-centred care and promoting independence

Salary: £48,000 - £60,000 per annum
