Join our team of experienced social workers and take on a challenging role as a Senior Practitioner Social Worker within our Community Mental Health Team. As a key member of the team, you will be responsible for assessing, planning, and reviewing the care and support of individuals with complex needs, including those with mental health difficulties, learning disabilities, and physical disabilities.

Main Responsibilities
  • Assess and plan the care and support of individuals with complex needs, working within a multi-disciplinary framework.
  • Undertake legal and statutory duties, including those required by the Care Act, Mental Capacity Act, and Mental Health Act.
Person Specification
  • Diploma or degree in social work
  • Qualified Social Worker with a live unrestricted registration with Social Work England
  • Evidence of Continuing Professional Development (CPD)
  • NVQ level 2 or equivalent standard of literacy and numeracy
Skills and Experience
  • Ability to work as an effective team member with excellent interpersonal skills
  • Good knowledge of Adult Safeguarding and its impact on service users and families
  • Experience of working across health and social care boundaries
