About the Role

We are seeking an experienced Mental Health Practitioner to join our County LMHT teams in Gedling. As a Community Psychiatric Nurse/Mental Health Practitioner, you will have the opportunity to develop skills needed to work within a community setting, providing care to an adult population across the Nottingham city area.

Key Responsibilities
  • Hold a caseload and work with patients and their families to support personalised care plans and provide specialist interventions.
  • Promote the recovery of patients and through working with partner agencies and primary care to ensure timely and safe discharge from secondary care.
  • Complete both triage assessments and more detailed initial assessments of new referrals.
  • Provide support via the teams duty system, either from telephone support or face to face.
Requirements
  • Professional Qualification (Mental Health) to degree level or equivalent.
  • Maintains current registration with the NMC.
  • Further specialist knowledge/post registration experience in specific service area or other formal training.
