Mental Health Practitioner

We are seeking an experienced Mental Health Practitioner to join our County LMHT teams in Gedling. As a Community Psychiatric Nurse/Mental Health Practitioner, you will work with patients and their families to support personalised care plans and provide specialist interventions.
Key Responsibilities- Manage a caseload of patients living in the community and provide initial assessments and one-to-one interventions.
- Develop and implement treatment plans for patients, using specialist skills and knowledge to maintain professional competence and fitness to practice as a nurse.
- Establish and maintain robust communication networks with clients, carers, team members, and other healthcare and social care workers.
- Use interpersonal skills to diffuse potentially emotive situations while undertaking assessments and pathway planning.
- Professional Qualification (Mental Health) to degree level or equivalent.
- Maintains current registration with the NMC.
- Further specialist knowledge or post-registration experience in specific service areas or other formal training.
