Job Summary:

As a Social Worker in our Mental Health Team, you will play a vital role in providing support to adults experiencing mental health challenges. Your key responsibilities will include conducting thorough assessments, developing care plans, and coordinating with multidisciplinary teams to ensure the best outcomes for our clients.

Key Responsibilities:

• Conduct detailed assessments to determine clients' mental health needs and develop personalized care plans.
• Collaborate with a multidisciplinary team, including psychiatrists, psychologists, nurses, and other professionals, to ensure coordinated care.
• Provide immediate intervention and support during emergencies or crises, ensuring the safety and well-being of our clients.
• Keep accurate and timely records of assessments, interventions, and care plans, adhering to organisational policies and legal obligations.

Requirements:

• Relevant social work qualification and experience in adult social work, including assessment and care planning.
• Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and multidisciplinary teams.
